(I) fire accidents caused by improper use of liquefied gas
fire accidents caused by improper use of liquefied gas mainly refer to the phenomenon that the concentration of liquefied gas in the air exceeds the standard limit and deflagration is caused by open fire ignition. The crew's poor safety awareness and illegal use of liquefied gas are the root causes of such accidents on luhuangyu 1273 and luhuangyu 2055 in 2006. In the first accident, the liquefied gas tank used for cooking was placed in the forecastle and simply tied with ropes, while the liquefied gas stove was placed in the forecastle at random without any fixed measures. During the operation of the ship, the hull was easy to roll or pitch under the influence of wind and waves, resulting in repeated sliding of the liquefied gas stove. In the process of boiling water, once the water in the pot spilled, it would extinguish the flame and cause gas leakage. Xue in the case found that when the liquefied gas was extinguished, he ignited again without taking any measures, resulting in the accident. In the second accident, the crew member Zheng had smelled the obvious gas smell in the cabin, which directly affected the intrinsic safety flow limiting characteristics, but he did not take basic protective measures such as opening the door for ventilation and closing the liquefied gas valve. Instead, he directly ignited the leaked liquefied gas with the open fire of the lighter, which led to the occurrence of the fire accident
(II) fire accident caused by short circuit of electrical equipment in the engine room
at 17:00 on January 12, 2002, Lu Huangyu 3536 helped anchor with Lu Huangyu 3517 in the sea area outside the first village of Beizhuang. One person was left on duty on each ship. Sheng, the attendant of 3517, asked Zhuo, the attendant of 3536, to take vegetables from his ship and cook on 3517. At about 18:00, Zhuo was afraid that the walkie talkie would be stolen, so he went back to the ship, unloaded the antenna of the walkie talkie, put the walkie talkie in his bed, closed the doors and windows of the cab, and returned to ship 3517. At about 19:00, Mr. Sheng suddenly saw that the cab of ship 3536 was on fire, and thick smoke rushed out of the window. After two people tried to put out the fire, the fire was put out, but the cab wall, roof, bed and some instruments were burned
after analyzing the cause of the accident, it is concluded that the fire in the cab was caused by Zhuo's failure to cut off the power supply after removing the interphone, and the wire connectors were not insulated. (III) the fire accident caused by improper use of the stove was also the Shandong Huangyu 3536 fishing boat in the above example. After investigation, during the repair period of the ship, the crew directly filled the oil into the furnace when cooking with diesel oil. When there was too much oil, Li Le (a pseudonym) of Hefei GuoXuan high tech power source Co., Ltd. said that the oil leaked from the bottom of the furnace and flowed outward, igniting the plastic oil barrel next to the stove, and the leakage of diesel oil in the barrel expanded the fire, Fortunately, there were no casualties, but the hull was seriously burned and had no repair value
(IV) analysis of the fire caused by lightning strike on fishing boats the reasons for this kind of accident on Luhuang fishing vessel 0130 in 2005 are as follows: first, the person on duty on that day, Mao, did not undergo pre job skill training, which violated the provisions of the "measures of the people's Republic of China for the certification of basic training examinations for fishing vessel ratings", and Mao was not qualified to engage in fishery production on fishing vessels; The shipowner did not equip qualified crew in accordance with the provisions of the measures for the safety supervision and administration of fishing ports of fishing vessels in coastal waters of Shandong Province, which laid a potential safety hazard for the accident. Second, as the person on duty on that day, Mao did not fully comply with the rules on duty and neglected to look, violating the provisions of the 1972 international rules for preventing collisions at sea that "every ship should always use vision, hearing and all effective means suitable for the environment and circumstances at that time to maintain a regular look, so as to make a full estimate of the situation and collision risk". Mao did not find the danger in time at the initial stage of the fire. When the fire was out of control, he was awakened by thunder and the cries of other crew members. At this time, the best time to extinguish the fire had been missed
third, the fire-fighting measures taken were improper. When he found a fire on the ship, Mao first hit it with clothes and then splashed it with water. He did not take the fire extinguisher first to put out the fire, delaying the fire-fighting time. Fourth, we will pay more attention to consumers' experience. After the fishing boat returned, a large number of plastic ropes, cages and other combustibles without fire prevention measures were piled up on both sides of the ship. After the fire, these combustibles were ignited, which further worsened the fire situation. Fifth, Lu Huangyu 0130 ship was not equipped with lightning protection facilities, the lightning protection equipment on board was not perfect, the wire layout was unreasonable, and the joints were not firm
(V) the high temperature of the smoke exhaust pipe in the engine room caused a fire of inflammables. Luhuangyu 9064 returned from sea area 116 at 12:40 on March 18, 2000. At 1:00 a.m. on March 19, the ship sailed to community 6 in sea area 104 and found a fire in the engine room. At 2:00 a.m., all the crew members left the ship and the ship sank. According to the detailed investigation and evidence collection, the cause of the accident was analyzed. First, the two fuel tanks of the ship were connected with elastic polyethylene pipes with spiral steel wires embedded in the pipe wall, and the joints were bound with steel wires. At that time, the offshore wind force reached level 7-8 of the north wind, which aggravated the rolling amplitude and frequency of the ship during the voyage. The wooden cushion block used to fix the fuel tank loosened under the action of wind and waves, the fuel tank shifted, the fuel pipe joint fell off, and a large amount of fuel in the tank leaked out, splashed on the exhaust pipe of the main engine, causing the engine room to catch fire; Second, the duty personnel in the engine room were in the cab at the time of the accident, and they did not stay on duty as required. At the initial stage of the fire, they did not find the danger in time, which led to the further expansion of the accident. In addition to this case, there are also crew members who illegally bake clothes around the smoke exhaust pipe, resulting in a fire. (VI) fire accidents of other ships affecting this ship. Such accidents are prone to occur during the fishing moratorium in the summer season and the centralized berthing in winter. The author has read reports of similar accidents many times. Since there has been no such accident in the region, no specific case will be explained in detail here
